- 10% off orthodontic treatments
- No maximum limit
Membership NOT An Insurance Plan. The benefits include:
- No waiting periods
- No preexisting condition exclusions
- No yearly maximum
- No deductibles
- No need for pre-authorizations
- No denial of claims

Things to note
- This plan is a benefit for those who are uninsured and can only be used at our office, Cusp Dental Boutique
- The membership fee is due at the time of enrollment and is valid for one year from the date of enrollment
- Discounts for treatment cannot be applied to discounts or charge plans
- It is the sole responsibility of the member to maximize their benefits by arranging the appropriate appointments within the 12-month membership period
- If the member fails to use the plan, the member does not become entitled to a refund
- Membership rates may change on an annual basis
- When used for Invisalign treatment, the person being treated must remain a member for the duration of treatment
